GHHS Induction Form

The Gold Humanism Honor Society (GHHS) recognizes individuals who exemplars humanistic patient care and who can serve as role models, mentors, and leaders in medicine. Individuals selected to the GHHS are recognized and celebrated in a formal induction ceremony and given a certificate and pin.

Chapter advisors and liaisons, please complete the form below prior to your induction ceremony’s date to receive certificates and pins for your nominees.

Note: There is a 21-day turnover to ship certificates and pins to your institution. If your induction date is outside of the required 21-day period, please provide a FedEx account number.

Award Winners

If your institution participates in the Leonard Tow Humanism in Medicine Award, the faculty award winner should be one of the four elected GHHS faculty members. If your institution participates in the Student Clinician’s Ceremony, the residents who are awarded the Tow Award and the Humanism and Excellence in Teaching Award should also be inducted at your ceremony. Please include all award winners on the inductee spreadsheet.

Add new inductees to the GHHS member database

Chapter advisors/liaisons also need to manually register all new GHHS members, along with their current contact information and member type (student, resident, fellow, or faculty), in the GHHS database. This database is critical for ensuring all new inductees are recorded as GHHS members and receive future updates from the Gold Foundation.
